JOB TITLE:	Licensed Vocational Nurse
DEPARTMENT:	Home Health
JOB SUMMARY:	Provides services in accordance with the policies/procedures of the Home Health Agency. Assists with the implementation and evaluation of the care of assigned patients. Provides patient care as directed by the attending physician and in collaboration with health team members. The LVN is directly responsible to the supervising RN.

JOB QUALIFICATIONS:

A. Education: Graduate of an accredited School of Vocational Nursing. B. Personal Job-Related Skills:Effective oral and written communication skills. C. Licensure, Registry or Certification: Licensed Vocational Nurse currently licensed by the State of Texas. D. Experience: Prior Work Experience: One year of Licensed Vocational Nursing experience preferred. Medicare and/or Medicaid experience preferred. Technical Training: Current CPR certification. Physical and Mental Requirements: Duties of the position require extended periods of standing and walking. Also required is bending, stooping, twisting, reaching and lifting up to 50 pounds. Use of fingers, both hands required, hear conversational voice 15ft.-one ear, near correctable vision to 13”-16”, far vision correctable in one eye 20/20, 20/40 in other. Able to operate a motor vehicle. Occasional heavy lifting required when moving or supporting patients who may suddenly become weak or helpless.
